Prescription Safety Glasses Schedule

Aircraft Line Maintenance, TULE and DWH Base Operating Units have selected 3M as the supplier of prescription safety glasses for Aircraft Maintenance Technicians, Stores personnel, and Supervisory personnel.

Program Scope:

The Company will provide prescription Safety Glasses to Aircraft Maintenance Technicians, Aircraft Maintenance Stores personnel, and those management and support staff exposed to risk of eye injury in the workplace.

Provided glasses will meet ANSI standard of Z87.1 and OSHA 29 CFR 1910.133.

Program Guidelines:

Lens Selections;
Single Vision, Bi-Focal, and Tri-Focal and Progressive Prescription Safety Glasses will be provided at Company expense. Lenses will be made of Polycarbonate or Plastic materials. Glass Lens material is not permitted. Employees may upgrade their lens options at employee expense. Tinted, Photochromics, Transitions, and Polarized Lens are not permitted

Frames selection:
Base and Group A through D will be provided at Company expense. Permanent side shields are required and must remain attached or the company must be reimbursed for the price of the glasses. Employees may upgrade their frame options at employee expense. (see catalog)

Frequency:
1 pair every 2 years or as needed, with approval, if frames/lenses damaged during use or due prescription change.

Requirments:

Employee, through their supervisor will obtain a 3M order/authorization form for eyewear purchase. Forms are available through Jet Net and the 3M web portal. Employee must provide 3M form and a valid current prescription (less than 1 year old).

Additional Options:

3M will offer a second pair of glasses at AA pricing to employees wishing to purchase a second pair. Employee must obtain an additional 3M order/authorization form from the web portal to order personal pairs. These forms are marked “employee paid program”.

There is a $27.00 dispensing fee charged to the employee for Safety Glasses purchased and dispensed by vendors at off site locations under the “employee paid program”. No additional dispensing charged if ordered from “On-Site” opticians.
