NOTE:
- Line one is for Pre 11-29-2012 issues
- Line 5 is for wages and are Entitled to Priority
- If this form looks different then the one you have or sent, as lang as you had the appropriate fields filled out it should be fine. (there are at least two versions that I have seen)
