INSTRUCTIONS FOR FILLING OUT THIS BALLOT
- Remove your ballot from the outer envelope, and review the enclosed materials.
- After removing your ballot, and reviewing the enclosed materials, mark the appropriate box next to your choice on the ballot by completely filling it in. Vote for only one – the number permitted for the Vice President’s office. No write-in votes are permitted.
- Do not write on, or in any way deface the ballot or the ballot envelope, or make any mark on the ballot that will in any way identify who you are.
- When you have finished marking your ballot, separate it from the stub on the bottom of the ballot along the dotted line. Fold the marked ballot, place it into the solid envelope marked “SECRET BALLOT ENVELOPE”, and seal it.
- Place the stub, which was on the bottom of the ballot, into the double window business reply envelope SO THAT THE ADDRESS AND BAR CODE SHOW IN THE WINDOWS. Place the Secret Ballot Envelope behind the stub.
- Seal the double window envelope and mail. (No postage is necessary if mailed in the U.S.A.)
- All ballots must be received by 9:00 a.m. on March 16, 2016, or they will not be counted.
- If you lose your Business Reply Mail envelope, you may return the ballot using any stamped envelope to:2016 Special Election TWU Local 514 P.O. Box 580069 Tulsa, OK 74158-9918
- If you do not receive a ballot, lose your ballot or if your ballot gets destroyed, you may request a duplicate by calling TrueBallot at 888-513-5128 any time of the day or night. A new one will be sent by first class mail within one business day. Requests made later than March 11 may not be received in time to receive your duplicate ballot and return it.
