Please note, the New Hire process for the TWU sponsored Short Term Disability policy with the Standard Insurance Company is to enroll new members within 31 days of their date of hire without providing medical evidence (EOI). Please bring completed forms to the Union Hall for processing or fax to 918-437-0310. For any questions or concerns you may contact me at 918-384-9760.
Also, please note, medical evidence (EOI) will be required for all who opt out of the Standard plan but later decide to enroll or for those who enroll more than 31 days after their date of hire.
